- Family Medicine Physician Partner Track
Description
La Plata Family Medicine is an independent, physician-owned family medicine practice seeking a Board-Certified Family Medicine physician to join our team on a partner track with a clear path to ownership.
We are not hospital-owned. We are not corporate. We are a collaborative group of physicians who practice relationship-based medicine, share governance, and retain full clinical autonomy.
This role is designed for a physician who wants more than employment — someone ready to build equity, shape their practice, and become a long-term owner.
The Opportunity
- Full-time Family Medicine position (MD or DO)
- Partner-track pathway with consideration typically after two years (may occur earlier)
- Immediate access to established patient volume, including patients transitioning from a departing physician
- Approximately 1,500 Annual Wellness Visits available for panel development
- Shared call: approximately one week every six weeks (minimal after-hours volume)
- Four-day clinical workweek typical
- No OB or procedural requirements
- US-based applicants only (relocation assistance not provided)
Compensation Model
Transparent, productivity-based compensation (“eat what you kill”) with biweekly draws against collections and quarterly reconciliation. Physicians directly benefit from panel growth, clinical productivity, and practice sustainability. Partnership includes shared quality and incentive revenue.

Requirements
- Board Certified Family Medicine physician (MD or DO)
- Actively enrolled in Medicare (or willing to enroll immediately)
- Minimum three years of clinical experience preferred
- Strong interest in long-term partnership and ownership
- Comfortable with productivity-based compensation
- Values collaborative practice, autonomy, and community-based care
- Eligible for medical licensure in Colorado
